Inelastic Demand, Elastic Policy: Rethinking the Defensive Status of Healthcare Equities in Asian Capital Markets

Healthcare equities are conventionally classified as defensive on the grounds that inelastic demand produces stable cash flows and low systematic risk. This paper argues that the classification is conditional rather than intrinsic. In many Asian markets, the state is simultaneously the dominant payer, price-setter, and regulator of the healthcare sector, so that the price at which stable demand is monetized becomes a policy variable. Drawing on conceptual analysis and an integrative synthesis of literature on asset pricing, policy uncertainty, pharmaceutical pricing intervention, public finance, and behavioral finance, the paper distinguishes demand stability, a property of the sector, from cash-flow control, a property of the institutional setting. It proposes a typology of four exposure regimes defined by payer concentration and policy predictability, and it specifies a fiscal-transmission channel through which geopolitical energy shocks may reach healthcare valuations: in energy-importing economies that subsidize fuel, subsidy burdens narrow fiscal space, raise the likelihood of health-expenditure containment, and affect the revenues of state-paid firms. Because this channel operates through budget cycles, it may unfold after, and run counter to, the immediate defensive rotation into healthcare that accompanies such shocks. The energy shock associated with the 2026 war involving the United States, Israel, and Iran serves as an anchoring context rather than an object of empirical analysis. Five propositions are developed, concerning payer dependence, policy-uncertainty sensitivity, fiscal transmission, time-varying defensiveness, and classification-driven investor behavior; they are advanced as theoretical claims rather than empirical findings. The paper derives implications for portfolio construction, disclosure regulation, and the design of fiscal and pricing policy, and it outlines an agenda for empirical testing.
